The current public health crisis has exposed deep cracks in social equality and justice for marginalised and vulnerable communities around the world. The reported rise in the number of ‘do not resuscitate’ orders being imposed on people with disabilities has caused particular concerns from a human rights perspective. While the evidence of this is contested, this article will consider the human rights implications at stake and the dangers associated with using ‘quality of life’ measures as determinant of care in medical (...) decision-making and triage assessments. Background: Clinical investigation is a growing field employing increasing numbers of nurses. This has created a new specialty practice defined by aspects unique to nursing in a clinical research context: the objectives (to implement research protocols and advance science), setting (research facilities), and nature of the nurse–participant relationship. The clinical research nurse role may give rise to feelings of ethical conflict between aspects of protocol implementation and the duty of patient advocacy, a primary nursing responsibility. Little is known about whether (...) research nurses experience unique ethical challenges distinct from those experienced by nurses in traditional patient-care settings. Research objectives: The purpose of the study was to describe the nature of ethical challenges experienced by clinical research nurses within the context of their practice. Research design: The study utilized a qualitative descriptive design with individual interviews. Participants and research context: Participating nurses (N = 12) self-identified as having experienced ethical challenges during screening. The majority were Caucasian (90%), female (83%), and worked in outpatient settings (67%). Approximately 50% had > 10 years of research experience. Ethical considerations: The human subjects review board approved the study. Written informed consent was obtained. Findings: Predominant themes were revealed: (1) the inability to provide a probable good, or/do no harm, and (2) dual obligations (identity as a nurse vs a research nurse). The following patterns and subthemes emerged: conflicted allegiances between protocol implementation, needs of the participant, desire to advance science, and tension between the nurse–patient therapeutic relationship versus the research relationship. Discussion: Participants described ethical challenges specific to the research role. The issues are central to the nurse–participant relationship, patient advocacy, the nurse’s role in implementing protocols, and/or advancing science. Conclusion: Ethical challenges related to the specialized role of clinical research nurses were identified. More research is warranted to fully understand their nature and frequency and to identify support systems for resolution. (shrink)

Policies promoted and adopted for allocating ventilators during the COVID-19 pandemic have often prioritised healthcare workers or other essential workers. While the need for such policies has so far been largely averted, renewed stress on health systems from continuing surges, as well as the experience of allocating another scarce resource—vaccination—counsel revisiting the justifications for such prioritisation. Prioritising healthcare workers may have intuitive appeal, but the ethical justifications for doing so and the potential harms that could follow require careful analysis. Ethical (...) justifications commonly offered for healthcare worker prioritisation for ventilators rest on two social value criteria: instrumental value, also known as the ‘multiplier effect’, which may preserve the ability of healthcare workers to help others, and reciprocity, which rewards past usefulness or sacrifice. We argue that these justifications are insufficient to over-ride the common moral commitment to value each person’s life equally. Institutional policies prioritising healthcare workers over other patients also violate other ethical norms of the healthcare professions, including the commitment to put patients first. Furthermore, policy decisions to prioritise healthcare workers for ventilators could engender or deepen existing distrust of the clinicians, hospitals and health systems where those policies exist, even if they are never invoked. BackgroundEvolving medical technology, advancing biomedical and drug research, and changing laws and legislation impact patients’ healthcare options and influence healthcare practitioners’ (HCPs’) practices. Conscientious objection policy confusion and variability can arise as it may occasionally be unclear what underpins non-participation. Our objective was to identify, analyze, and synthesize the factors that influenced HCPs who did not participate in ethically complex, legally available healthcare.MethodsWe used Arksey and O’Malley’s framework while considering Levac et al.’s enhancements, and qualitatively synthesized the evidence. We searched (...) Medline, CINAHL, JSTOR, EMBASE, PsychINFO, Sociological Abstracts, and ProQuest Dissertations and Theses Global from January 1, 1998, to January 15, 2020, and reviewed the references of the final articles. We included articles written in English that discussed the factors that influenced physicians and registered nurses (RNs) who did not participate in end-of-life (EOL), reproductive technology and health, genetic testing, and organ or tissue donation healthcare areas. Using Covidence, we conducted title and abstract screening, followed by full-text screening against our eligibility criteria. We extracted the article’s data into a spreadsheet, analyzed the articles, and completed a qualitative content analysis using NVivo12.ResultsWe identified 10,664 articles through the search, and after the screening, 16 articles were included. The articles sampled RNs (n = 5) and physicians (n = 11) and encompassed qualitative (n = 7), quantitative (n = 7), and mixed (n = 2) methodologies. The care areas included reproductive technology and health (n = 11), EOL (n = 3), organ procurement (n = 1), and genetic testing (n = 1). One article included two care areas; EOL and reproductive health. The themed factors that influenced HCPs who did not participate in healthcare were: (1) HCPs’ characteristics, (2) personal beliefs, (3) professional ethos, 4) emotional labour considerations, and (5) system and clinical practice considerations.ConclusionThe factors that influenced HCPs’ who did not participate in ethically complex, legally available care are diverse. There is a need to recognize conscientious objection to healthcare as a separate construct from non-participation in healthcare for reasons other than conscience. Understanding these separate constructs will support HCPs’ specific to the underlying factors influencing their practice participation. (shrink)

Over the past decade, genetic tests have become available for a wide variety of disorders. As a result we are able to predict, with some degree of certainty, whether or not an individual will develop such diseases as breast cancer, Huntington's disease, polycystic kidney disease, and familial adenomatous polyposis. The ability to predict disease poses several unique ethical considerations for clinical decisionmaking regarding the provision of genetic testing. Patients must be able to comprehend the complexities of genetic testing and the (...) potential meaning of the results. Patients must consider the emotional, social, and economic consequences of revelations regarding their risk status. A sample of college women and men responded to a survey assessing attitudes, beliefs, experiences, and behaviors relevant to sexual harassment and assault. Men were more tolerant of sexual harassment, more likely to believe that heterosexual relationships were adversarial, more likely to subscribe to rape myths, and more likely to admit that they might sexually assault someone under some circumstances. Data from the present study support the proposition that relevant affective, cognitive, and behavioral indices of hostile sexuality directed against women (...) are linked, supporting the assumption of a continuum of misogyny. Significant positive correlations were predicted and found among men's self-reported tolerance for sexual harassment, adversarial sexual beliefs, rape-myth acceptance, likelihood to rape, and experience as a sexual victimizer. Among women, belief and attitude measures were positively correlated but, as predicted, experience of sexual victimization was not reliably related to any other measure, supporting the conclusion that personal characteristics are not relevant to college women's sexual victimization. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. This study investigates the relationship between mental health and emotional intelligence among senior high school students in a public school. Thus, the study employed a correlational design to measure the relationship between mental health and emotional intelligence among 152 Grade 12 senior high school students in a public school. Hence, to measure the study’s variables - Mental Health Inventory and Emotional Intelligence Scale (EIS) were utilized. Based on the inferential statistics, the r coefficient of 0.32 indicates a low positive correlation (...) between the variables and a p-value of 0.00, indicating a significant relationship between the respondents' mental health and emotional intelligence. When parents must rely on public assistance and family shelters to provide for their children's most basic needs, they lose autonomy. Within a system of public assistance that already stigmatizes and isolates its beneficiaries, their family lives become subject to public scrutiny and criticism. They are _parenting in public._ This book is an in-depth examination of the realities of life for parents and their children in family shelters. PunitheologyThe explanations started pouring in even before the virus attained “pandemic” status in March of 2020: we were being punished. According to a vocal subset of Evangelical pastors and ultra-Orthodox rabbis, the death-dealing virus was divine retribution for the sins of (who else?) LGBT-identified people and their allies, who aggressively violated what the pastors and rabbis called (...) “the order of nature.”1 Meanwhile, their left-leaning counterparts argued that the sin in question wasn’t so much sexual as ecological: in the words of one Roman Catholic commentator, the coronavirus was God’s punishment for “our unfettered domination attitude toward nature.”2At the same time, an equally vocal throng of secular environmentalists called the virus nature’s own punishment for our manifold violations against her—in particular, the extractivism, pollution, alleged overpopulation, deforestation, carbon emissions, and industrial slaughterhouses that allowed the zoonotic strain to break out among humans in the first place.3 Depending on whom [End Page 5] you ask, then, the disaster we’ve come to encapsulate as COVID-19 is either godly or natural retribution for our manifold sins against “nature.”Although these sacred and secular punitheologies are not equivalent, they bear striking similarities to one another thanks to the monotheistic heritage of the concept of “nature” itself. As we will see, whether it is invoked by pastors, politicians, activists, or scientists, “nature” in the Western-descended world is presented as singular, nonwhite, and feminine; morally normative; and prone to abuse. Nature is often capitalized, often maternalized, and reliably distinguished either from a disembodied male God, a universalized “humanity,” or both. Nature, in short, carries immense ideological freight. Specifically, it installs and polices the work of what Kelly Brown Douglas calls “theo-ideology”: a “sacred legitimation” for a social order that seeks to present itself as unquestionable.4 And “nature” does this unparalleled ideological work thanks to the freedom it keeps promising from ideology: “your” way of being might be constructed, distorted, historical, subjective, contingent, artificial, cultural, or indeed sinful, but “ours” is natural—which is to say universally, even transcendently, true.Whether it comes from the mouths of angry clerics or righteous ecologists, then, the call to get right with nature is a call to adhere to one or another self-concealing social arrangement—to a set of interests and values whose power derives from its seeming so “natural.” For this reason, many contemporary thinkers recommend abandoning the term altogether. Donna Haraway suggests we try the non-binary and carefully pluralized “naturecultures”; Bruno Latour proposes “critical zones,” and Timothy Morton pivots to ecological practice which, he insists, can only function if we let go of the romantic delusion of “nature.”5Regardless of nature’s conceptual integrity, it is important to attend to the term in order to account for its rhetorical force and ubiquity—for the polyphibious presence of “nature” within the left and right-wings of science, religion, and politics alike. In the process, I hope to unearth some of the mythic norms of race and gender that this concept both entrenches and conceals in the American imagination. Finally, I will ask what happens when we start messing with these categories, beginning with the mirroring chasms between humans and nature, and nature and God. How might we start to think pantheologically about this unrelenting pandemic, and why would anybody want to?II. Mary Wollstonecraft's Vindication of the Rights of Woman, first published in 1792, has proved a problematic ‘classic text’ for feminism. This paper focuses on the liberal concept of self‐ownership to show how the Vindication both confronts and perpetuates the dilemmas of ‘liberal feminism’. Self‐ownership is not a term used by Wollstonecraft herself, but I make use of it in this paper because I believe it captures what she means by ‘independence’, arrived at by a combination of reflection, self‐government and labour. (...) It implies a natural right to resist oppression and arbitrary government and so reaches the heart of Wollstonecraft's argument against patriarchy as a particular form of arbitrary power. As Alan Ryan argues, ‘each of us is obliged to obey the government so long as it upholds our natural rights, but if it violates them, we are not obliged to obey’. As a liberal concept, self‐ownership is fraught with ambiguity for feminism. Donna Dickenson argues that it can be interpreted both as ‘masculinist through and through’ and as ‘the essence of feminism’. Its very ambiguity makes it an appropriate conceptual lens through which to view Wollstonecraft's own contradictory arguments. (shrink)

Discussion of Wittgenstein's Tractatus is currently dominated by two opposing interpretations of the work: a metaphysical or realist reading and the 'resolute' reading of Diamond and Conant. Marie McGinn's principal aim in this book is to develop an alternative interpretative line, which rejects the idea, central to the metaphysical reading, that Wittgenstein sets out to ground the logic of our language in features of an independently constituted reality, but which allows that he aims to provide positive philosophical insights into (...) how language functions. McGinn takes as a guiding principle the idea that we should see Wittgenstein's early work as an attempt to eschew philosophical theory and to allow language itself to reveal how it functions. By this account, the aim of the work is to elucidate what language itself makes clear, namely, what is essential to its capacity to express thoughts that are true or false. However, the early Wittgenstein undertakes this descriptive project in the grip of a set of preconceptions concerning the essence of language that determine both how he conceives the problem and the approach he takes to the task of clarification. Nevertheless, the Tractatus contains philosophical insights, achieved despite his early preconceptions, that form the foundation of his later philosophy. -/- The anti-metaphysical interpretation that is presented includes a novel reading of the problematic opening sections of the Tractatus, in which the apparently metaphysical status of Wittgenstein's remarks is shown to be an illusion. The book includes a discussion of the philosophical background to the Tractatus, a comprehensive interpretation of Wittgenstein's early views of logic and language, and an interpretation of the remarks on solipsism. The final chapter is a discussion of the relation between the early and the later philosophy that articulates the fundamental shift in Wittgenstein's approach to the task of understanding how language functions and reveal the still more fundamental continuity in his conception of his philosophical task. The topic of critical thinking has engaged philosophers, psychologists and educators for well over one hundred years. Amid polarized political attacks on the teaching of controversial issues, however, education in critical thinking appears to be nearing a new low, not only in the United States, but also in other countries being torn by partisan politics. This article reviews the ebb and flow of critical thinking efforts, suggests explanations for their discouraging results, and proposes a way forward that treats critical thinking (...) as an integrative process advanced by Mary Parker Follet, and more recently espoused by Bruno Latour and Donna Haraway. In the exploratory spirit of John Dewey and Alfred North Whitehead, I am introducing this essay, not with analysis or theory, but with a narrative of educational events in all their richness of context. These events describe how middle school students addressed the question of whether wolves should be removed from the endangered species list in the Northern Rocky Mountains of the United States. In the course of this discussion, I wish to demonstrate that thinking critically requires moving beyond winning, losing, or compromising to resolution of conflict, and furthermore, understanding that even stable resolutions must be revisited as events change. The article explores how digital images of hands are used as a symbolic representation of intimacy and intimate emotions in influencer communication on Instagram. Based on digital ethnography with female influencers in the Czech Republic, the analysis focuses on three categories of communicative practices, where hands function as a visual representation of intimacy—creating community, a sense of vulnerability, and the notion of rawness and openness. The analysis points to the gendered nature of influencer communication. It explores how intimacy is established (...) specifically by women influencers who need to navigate vulnerability with the need to protect themselves against gender-based online violence in the form of hate comments and sexualized hate speech. How is it that ordinary people decide to take part in genocides? Philosophers and psychologists have attempted to provide explanations of how genocidal organizations bear on the moral judgement of genocide participants. Here, I resituate those findings within the field of human resource management. I highlight how basic principles of management and HR management can lead ordinary individuals to judge their participation in a genocide as acceptable. Although initially only designed to increase motivation and productivity, these techniques also affect individuals’ (...) awareness of ethical issues. Consequently, participants shift their judgements in favour of the genocidal organization, forgetting the victims. Management studies have seldom addressed the topic of genocide, despite clues in the literature that genocides are organizational in nature. By combining the three fields of management, business ethics and genocide studies within an approach based on transdisciplinary analysis, I hope to show that genocides constitute a legitimate subject for management studies. The concept of community of persons, which focuses on both persons and the whole, helps understand solidarity. The latter is based on the social nature of persons. Community of persons and solidarity seems to be able to move away from the individualist perspective or the individualism-collectivism dichotomy. Using autopraxeography in a pragmatic constructivism epistemological paradigm, this article aims to explore how organizations can develop solidarity in a workplace. The experience presented takes place in a bank. It shows that communities of (...) persons with employees and customers are both ethical and financially efficient. These communities build a dialogue between persons and organizations. Nevertheless, it is impossible to force solidarity because it could generate derision that is contrary to the wished goal. Finally, while this model is based on solidarity, it focuses solely on internal solidarity. (shrink)

Lack of attribution and plagiarism can create a special problem for journalists. As numerous examples indicate, there is confusion about the sometimes fine line between lack of attribution and plagiarism. But there is even more confusion over how to solve the problem. Short of restructuring the journalism profession to create an overall governing body similar to the law bar, there is no way to create a set of national guidelines on when lack of attribution might become plagiarism. C’est à partir de deux exemples concrets, celui des employés de banque et des chauffeurs de taxi collectif à Beyrouth que nous nous proposons d’interroger l’« absence » de la question du temps de travail dans les revendications sociales au Liban. Une absence qui serait l’indice de la prégnance d’un autre rapport au temps : on serait en présence de régimes de temporalités hétérogènes les uns aux autres, à la mesure de la fragmentation de la société entre des mondes sociaux (...) où le travail est soumis à des contraintes radicalement opposées. Dans le cadre des visual studies, Mitchell a montré que l’image organise de plus en plus notre perception sémiotique, en réarticulant la production du sens dans une perspective iconique. L’hypothèse d’un « devenir-image » du texte, du discours et de l’interaction n’est donc pas nouvelle mais semble confirmée par la communication numérique, où l’image joue un rôle important, réorganisant les contenus de sens et prenant parfois le pas sur l’expression verbale au sein du technographisme, défini comme une production multimédiatique numérique (...) native articulant image, texte et éventuellement son. Dans cet article, je propose de mettre en œuvre cette hypothèse à partir de l’exemple de la photographie de texte imprimé ou sur écran, qui est de plus en plus intégrée dans les discours numériques natifs, en particulier sur les réseaux sociaux comme Twitter ou Facebook. Je montrerai qu’elle relève d’une iconisation du texte, témoignant d’une conversion numérique de la scripturalité et posant la question d’un pilotage contemporain du sens par l’image. (shrink)
